What are your office hours?
We are open Monday from 7:30 AM to 12:00 PM and 2:00 PM to 4:00 PM, Tuesday and Wednesday from 7:30 AM to 12:00 PM and 2:00 PM to 6:00 PM, Thursday from 9:00 AM to 12:00 PM and 2:00 PM to 6:00 PM, and Friday from 7:30 AM to 12:00 PM. We are closed Saturday and Sunday.
How do I book my first appointment?
Fill out the form on this page with your first name, email, and phone, and a member of our team will be in touch shortly. You can also call (630) 513-7770 directly. New patients start with the $49 New Patient Visit, and same day appointments are often available.
What should I bring to my first visit?
Bring your completed patient forms, which you can download from the Patient Forms link in our menu. If you are using insurance, bring a copy of your insurance card and have your date of birth ready so our billing team can run a benefits check before your appointment.
